Scismic is supporting the growth of program created by the Maryland Department of Health which includes a coalition of healthcare and aging service advocates, leaders and community organizations. This program's mission is to serve frail older adults in the West Baltimore community to lead connected, meaningful lives.

The Physical Therapist role offers a unique opportunity for a new elderly-focused program in West Baltimore which consists of a framework of supportive, innovative, and expert leadership team and company. Together with their coalition of strong partners within the community, the Physical Therapist will work closely with the Rehabilitation Department and Social Services to carry out our clinical model of care, and have a transformative impact on a particularly under-served community in West Baltimore.
Seven core principles drive our approach to care. These include:

They support elders with cognitive and physical disability to sustain meaning in their daily lives. They form close community partnerships to help elders engage outside our centers.
- Respect the Right to Independence: The organization aims always to follow our participants’ wishes. Wherever possible, participants maintain control over all decisions — from medical procedures to what to eat for dinner.
- Pay Attention to What Is Working Well: The goal is to multiply the days each participant is glad to be alive. Participants are encouraged of positive experiences of daily life, supporting the things that matter most to them.
- Provide the Right Amount of Medical Care: Elders often have many diagnoses, medications and specialists, involving complex care decisions. The team helps participants understand their choices. They deliver thoughtful, appropriate and personalized care, doing what our participants want — nothing more and nothing less.
- Guide All the Care Provided: The team always follow and guide our participants’ care — in their homes, at our center, in the hospital, and everywhere.
- Live with Our Communities, Not Just in Them: The organization's work in communities where aging in place is valued and integrated care is a shared mission. The team builds deep relationships with community organizations (see video) beginning long before we open our doors. We aim to reach beyond the walls of the center to bring community members in and participants out for person-to-person connection.
- Employ Technology Thoughtfully, Not Aggressively: The team uses technology because it can enrich the lives of participants, improving care and connecting family members with their loved ones and their care providers. But technology is only used to complement — never replace — direct human interaction.
Main Responsibilities:
- Delivery of appropriate physical therapy evaluation and treatment to program participants on acute, restorative, or maintenance levels as needed in the facility or in the home.
- Performs timely initial and annual assessments for all participants and corresponding plans of care. Performs semi-annual reassessments and unscheduled change of condition assessments for all participants receiving physical therapy services as well as corresponding plans of care
- Participates in home safety evaluations as directed for potential enrollees into the program
- Responsible for updating care plans and measuring the outcomes for physical therapy goals within the care plan at least every 90 days
- Assists in the delegation of therapy plans for specific participants to Physical Therapy Assistants according to complexity of participant’s needs and nature of required physical therapy procedures
- Trains family members, caregivers and other staff members in methods to enhance mobility, safety and functional independence of participants
- Will support the success of the entire team by promoting a collaborative work environment
- Consistently contributes to problem-solving
- Completes annual trainings as required
- Serves as a required member of the Interdisciplinary Team (IDT) and attends daily IDT meetings
Qualifications:
The organization is committed to building a diverse and inclusive organization. For our Physical Therapist role, we are looking for the following qualifications:
- Graduate of a physical therapy program accredited by the American Physical Therapy Association
- Possess a Physical Therapist License from the PT Board of Maryland
- Must submit to and pass a pre-employment drug/alcohol screening and criminal background check
- Must have a current Driver’s License and/or be able to commute to the office and participant home visits
- Up-to-date vaccinations, including COVID vaccine, required
